Print and Complete These Forms:
In preparation for getting a new job, your student employment file must be complete. Before you can begin work, you must complete and submit all of the following forms to the Financial Aid Office.
- I-9 Form -- Employment Eligibility Form
- W4 Form -- Federal Withholding Form
- IT-2104 -- New York State Withholding Form or IT-2104E -- if you are EXEMPT from taxes.
- FERPA Non-Disclosure Agreement
- Federal Work Study Rights & Responsibilities Fact Sheet
- Federal Work Study Student Information
